- Valid CNIC of all family members who are above 18 years of age
- Form B for children who are under 18 years of age
- Utility bills (electricity, gas, or water) to prove your residency
- Disability certificate (if applicable)

How to Register for BISP Dynamic Survey 2024
For the registration of BISP April Dynamic Survey 2024, please visit your nearest Ehsaas BISP Tehsil Office and follow this procedure.
- Go to the information desk and join the queue.
- When it’s your turn, provide your CNIC or the CNIC of any family member.
- If you have already been surveyed and need to update your roaster, you will be issued a token for Roaster Update.
- If you haven’t been surveyed yet, you will be issued a token for New Survey.
- Proceed to the Data Entry Desk/Counter and wait for your turn as per the token number issued.
- At the Data Entry Desk, provide the CNIC of all household members, B Form of all children in your family
- Answer the questions asked by the Data Entry Officer (DEO) regarding your household members’ details, education, health, assets, income, etc.
